Submitting forms to WIPO Madrid not working

Hello

When users are trying to upload a form and enter the information, such as: form type, reference etc.  the information disappears.  Users have tried different browsers such as Google Chrome and Internet Explorer but this has not worked.

Please help why forms cannot be loaded?
